Emergency Contact Information

Authorization for Emergency Medical Treatment

In the event of an emergency, I authorize Maniacal Kids to seek and obtain any necessary medical treatment for my child. I understand that every effort will be made to contact me or the emergency contact listed above prior to any medical treatment.

Release of Liability

I understand and acknowledge that Maniacal Kids will take reasonable precautions to ensure the safety and well-being of my child during the time they are in their care. However, I recognize that accidents and unforeseen circumstances may occur. Therefore, I release Maniacal Kids, its employees, agents, and volunteers from any and all liability for any injuries, damages, or losses that may occur to my child while under their care.

Photography Release (Optional)

I hereby grant permission for photographs and/or video footage of my child to be taken during the childcare period for promotional or informational purposes related to Maniacal Kids, unless otherwise specified in writing.
